User Communication Meetings¶

Forms¶

LNF Visitor Form: Use this form if you wish to bring a visitor into the lab. Note that the visitor access is intended for a lab tour or similar type of visit. No processing or training can occur.

Instead of the form, you can send the following information to LNF-access@umich.edu:

• name, affiliation and contact information for escort (LNF user bringing a visitor);
• name, affiliation and contact information for visitor;
• type of access requested (cleanroom, wet chemistry etc);
• date and duration of the visit.
  

You will then be able to pick up a visitor badge from one of the LNF staff members.
